Health Survey

You have been selected by your GP surgery to take part in this Health Survey being conducted by Three Rivers District Council.
The aim is to improve awareness and access to health and support services available in the local area. This survey will go through a series of questions on different areas that can impact on your overall health. You will be offered the option to receive more information on support available in the local area, or to receive a referral directly into a service.
